Investigation Reports

In meeting its statutory requirements to “improve accountability and transparency in the child protection system and promote better outcomes for children and families involved in the child protection system,” as stated in C.R.S. 19-3.3-101(2)(a), the CPO will provide the public and stakeholders any recommendations it makes to an agency and/or provider. The CPO will do so by publicly releasing its investigation reports.

11839 Adams County Human Services Department The CPO received a complaint concerning an open case with the Adams County Human Services Department. The complainant stated that they were concerned the caseworker was not checking in on the child and there was poor communication between the placement provider and the caseworker. 11875 Denver Department of Human Services The CPO received a complaint concerning the Denver Department of Human Services (DDHS). 2016-2243 Pueblo County Department of Social Services The CPO received a complaint concerning the Pueblo County Department of Social Services’ (PCDSS) handling of a report involving a 2-year-old girl. 10133 Montezuma County Department of Social Services The CPO received a complaint concerning the death of a child in Montezuma County. The complainant stated that the Montezuma County Department of Social Services (MCDSS) had a history with the family and had previously provided inadequate responses to reports of abuse in the home. 10991/11037 Denver Department of Human Services and Arapahoe County Department of Human Services The director of the Denver Department of Human Services (DDHS) contacted the CPO after DDHS staff discovered multiple inconsistencies in a caseworker’s recorded documentation. The caseworker’s work did not appear to accurately reflect information that was received from parties in a case involving the death of an infant.
